Extended Curriculum Enrollment Form Student Contract & Enrollment
I will abide by all N.F.P.T. Policies and Procedures. All monies must be paid in full prior to sitting for the final
Nationally Accredited Certification in the amount totaling $1,229.00.
Upon enrollment student will receive Personal Trainers Manual, CD-ROM containing Power Point presentation, welcome e-mail
with attachments to print, Pre-Workshop Assignments.
Students are responsible for printing materials for the workshop. If student does not print and bring materials to
the workshop, the NFPT instructor will have copies of power point presentation available for $6.00 each and $3.00 each
for Day 2 workshop Lab Practices copies.
If choosing the finance option, a minimum of $375.00 must be paid upon enrollment, $375.00 on test date, and full
balance due of $479.00 due on attendance of class seven ( principles of lower body stretching). Finance charges will
result in the additional charges of $102.48 will be added to final payment. Finance charges are derived at 12% of $845.00
balance due after enrollment deposit
I understand that there is a no-refund policy as part of this enrollment. I will be expected to have full knowledge
of the N.F.P.T. trainer's manual prior to the hands-on training workshops that I am entitled to attend as part of this enrollment.
I will participate in four (6) workshops each consisting of 2 1/2 hours hands on training.
I will have an additional (6) training sessions to learn advanced workout design and core stability training, and
stretching techniques.
I understand that the examination is a closed book examination and that failure to abide by these rules can and will
forfeit my eligibility and all monies paid to sit for this examination.
I understand that I will be allowed a maximum of two hours to complete this examination.
Program must be completed within six months from date of enrollment. Failure to complete the program within allotted
time will result in forfeiture of all monies paid and registration will have to be renewed.
I understand that in the event I do not pass the test, an additional retest fee of $80.00 will be assessed as well as an
optional fee of $150.00 to re-attend the Workshop.
